Damage Claim Form

Please complete the following Damage Claim Form within 20 days of your purchase,  provide your order number, a picture of your packing slip, and a picture of the defected box, and item. Please make sure you do NOT throw away the packaging, as we need photos of this as well. Tickled Pinque must examine the product for defects prior to providing a replacement product or product credit.
